An OSHA recordable injury or illness generally must be entered on Forms 300 and 301 when it is work-related and meets Part 1904 criteria—such as death, days away from work, restricted work or transfer, medical treatment beyond first aid, loss of consciousness, or a significant injury/illness diagnosed by a physician or other licensed health care professional.
